How to Activate Your Patient Portal
If you haven’t already, you will receive an email invitation from yourlife@newtownathletic.com to activate your YOUR.Life Clinical Patient Portal.
Please click the link in the email and follow the prompted steps to set your username and password.
If you have already set your username and password, click the button below to the Patient Portal log in page.
To re-initiate the portal invitation, follow the steps below:
- Click the button above here to be redirected to the Patient Portal log-in page.
- Click I'm a registered patient. Create my account!
- Enter the required verification information.
How to Complete Forms
Prior to your appointment, please complete all required forms on your Patient Portal. Below are steps on how to locate and complete forms.
- Log into the Patient Portal (instructions above)
- Once logged in, on the side panel, navigate to the Forms & Questionnaires tab
- Locate RESULTS + ROAPMAP: Medical History & Health Goals Form
- Click the large, red START button.
- Complete the form and hit SUBMIT at the bottom of the form.
PLEASE NOTE: Health History form must be completed in order to receive your Results & Road Map (either delivered digitally or at your Results & Road Map in-person appt).
Dr. Meg and her team prepare your Road Map of personalized recommendations and actionable next steps, based off of your labs and the completeness of this form.
